Oregon Community Birth Transfer Partnership
Below are links to the flyers with QR codes and links for the Community Birth Transfer Partnership surveys. There are surveys for clients, community midwives, receiving providers, nurses and EMS. These surveys are critical in our work to improve the transfer experience for everyone involved. Some hospitals put the flyer up in break rooms or by the nurses station for easy access and also post in patient rooms. Community midwives are also encouraged to share the client survey with their clients when a transfer occurs.

The Oregon Midwifery Council and the Oregon Perinatal Collaborative work together to improve home birth and birth center to hospital transfers across Oregon through the Community Birth Transfer Partnership. This statewide improvement initiative was founded in 2021 and is open to all birthing hospitals, community midwives, and EMS services in the state.
The Community Birth Transfer Partnership relies on ongoing feedback through surveys about transfer experiences from mothers/birthing people and providers. Our goal is for every person who experiences a community birth transfer to receive a survey from their midwife or from the hospital and for community midwives, receiving providers and nurses, and EMS personnel to fill out a survey every time they have a transfer.
